10 Add a submit button for the form by doing one of the following:

• In the Insert bar’s Forms category, click the Button button or drag it from the Insert bar to

the table cell.

• Choose Insert > Form Objects > Button.

A Submit button inserts in the document. In the Property inspector, the button is already

labeled and the Action is set to Submit form.

11 Repeat step 10 to add a another button to the form. You’ll add a reset button to let a user reset

the form if they need to..

12 In the Property inspector, for Action, select Reset form.

13 In the Button Name text box, enter Reset.

Your form is now complete and should look similar this:

14 Save your document.
